Facility Assessment

As with our in-home service, our facility based service offers superior proactive assessments to clients living in retirement homes or with-in long term-care facilities. Regular visits from a qualified health care professional to monitor and assess health on an on-going basis provides the extra assurance and support that many people need and want. These assessments also provide updated suggestions and recommendations on current health status and care needs that can dramatically enhance quality of life.

Our-facility based service offers you and your family on-going expert insight and perspective on your personal health state or that of your loved one. We will work diligently to make healthcare simple to understand for everyone within the circle of care. Similar to our in-home service model, visits can be scheduled on a monthly basis or more frequently if required.
