Service Overview

Initial Visit

During this introductory visit, our staff member will conduct a thorough physical and needs assessment. The baseline data and relevant observations and findings will be logged into our secure online client portal. In addition, any necessary health and environmental recommendations will be communicated directly to the client and family via this system. You will be provided access to review all discussed information anytime, as well grant access to current authorized care providers (i.e. physicians, pharmacists) to further enhance your personal or loved ones overall level of care.

Follow-up Visits

At each scheduled follow-up visit, the designated health care professional will re-assess the client's physical condition and any required changes in health care needs. Our staff will explain all findings and observations to the client and family, and update this data on the on-line client portal for future reference. The client's primary medical physician, specialists, pharmacists and ancillary health care providers can also review this information anytime to stay informed and to troubleshoot any areas of concern.
